The AI CFO Is Now a Friction Manager, Not Just a Cost Manager
Last month, I joined other healthcare leaders on ‘The AI CFO’ panel at Netsmart CONNECTIONS. It was a vibrant conversation about balancing cost, investment, and governance, but one concern is top of mind: the upcoming Medicaid changes.
The Medicaid changes raise the financial stakes
One of the most discussed topics on the floor was the upcoming change to Medicaid eligibility and retroactive billing under the One Big Beautiful Bill Act. Retroactive coverage, the window that lets providers recover payment for care already delivered before a Medicaid application is approved, is shrinking. Most states currently allow up to three months. Starting January 1, 2027, that drops to one month for expansion enrollees and two months for traditional enrollees, and roughly a dozen states have already shortened or eliminated it.
Automation and AI close that gap
For providers, that is a direct financial risk, and the leaders I talked to at CONNECTIONS knew it. Every day between date of service and confirmed eligibility now carries more exposure than it did a year ago. A verification delay that used to be recoverable within a three-month window has far less room to correct itself, and for services delivered near the edge of that window, the cost of a slow process shifts from a rounding error to a write-off.
AI agents, combined with automation, can help with this:
01 Eligibility Verification Agents
These agents monitor the schedule ahead of a visit, check payer coverage, and flag problems (like an expired plan or a changed eligibility status) before the patient arrives, not after the claim is filed. That shifts discovery from months after service to hours before, which is exactly what providers need as the retroactive window closes. While financial risk doesn’t disappear, it’s front-loaded, meaning a team member can handle any exceptions with care.
02 Prior Authorization and Denial Management Agents
The Prior Authorization and Denial Management Agents review payer policy, complete and submit authorization requests, and automatically track status. When a denial does happen, they identify the root cause, prioritize by revenue impact, and draft the appeal. That’s work that otherwise falls to staff who call payers and assemble appeal packets by hand. Faster authorization and faster appeals both shrink the same window that Medicaid changes are tightening.
Remove the friction, not just the cost
The real theme running under every session at CONNECTIONS was friction: the gap between something happening- a visit, a policy change, a denial- and the organization knowing about it and acting on it. The Medicaid changes just made that gap more expensive to leave open.
